ALBASAR INTERNATIONAL FOUNDATION-INVITATION TO TENDER FOR THE SUPPLY OF MAJOR OPHTHALMIC EQUIPMENT

SUPPLY OF MAJOR OPHTHALMIC EQUIPMENT

ALBASAR INTERNATIONAL FOUNDATION invites qualified suppliers to submit quotations for the supply, delivery, and installation of the major ophthalmic equipment listed in Section 5, for use in a Secondary Eye Centre and Vision Centres.
The equipment comprises seven (7) items:

1. Compact Ophthalmic A/B Scan Ultrasound System
2. Computerized Non-Contact Tonometer
3. Automated Visual Field Perimeter – Type A (Full-Field, Goldmann Bowl)
4. Automated Visual Field Perimeter – Type B (Compact, 60° / 160°)
5. Auto Kerato-Refractometer
6. Slit Lamp
7. Ophthalmic Operating Microscope (Floor-Standing)

2.1 Eligibility and documents to accompany the quotation

a) Bidders shall be legally registered suppliers of medical/ophthalmic equipment. Attach: certificate of incorporation (CAC), valid tax clearance certificate, and evidence of tax registration.

b) Attach a valid Manufacturer’s Authorisation Letter (or authorised-distributor certificate) for each item offered, confirming that the bidder is authorised to supply, install and service the product in Nigeria.

c) Attach evidence of regulatory registration or approval of the offered products where applicable (for example NAFDAC medical device registration) and, for electrical equipment, conformity to recognised standards (CE, ISO 13485 manufacturer certification or equivalent).

d) Attach at least three (3) references (purchase orders or completion letters) for similar supplies made within the last three (3) years.

e) Attach the original manufacturer’s technical brochure or datasheet for every item offered. Offers without technical literature may be rejected.

2.2 Preparation of the quotation

a) Quotations shall be in English and shall be submitted using the Price Schedule in this document. Every line item must be priced; state the make, model and country of origin of each item offered.

b) Prices shall be quoted in Naira, fixed for the validity period, and shall be inclusive of packing, freight, delivery to the delivery location(s), and (where applicable) installation, any applicable taxes shall be shown separately.

c) Quotations shall remain valid for sixty (30) days from the closing date.

d) The specifications in this RFQ are minimum requirements. Equivalent or superior products are acceptable, provided the bidder demonstrates equivalence in writing. Any deviation from the specification must be clearly stated in the quotation.
